Important Information Before You Begin

Only one prerequisite course may be taken concurrently with CHDV 4950/4980 in the same term. Therefore, if you indicate that you have not completed more than one prerequisite, please wait until you meet this requirement before applying for the course permit. When submitting your transcripts (CSULA and any applicable community college transcripts), please highlight the completed prerequisites and email them to the fieldwork coordinator (as directed on the last page). Applications without proof of course completion will not be processed. Thank you.
